When circumstances change suddenly
An unexpected change can bring a lot of decisions at once. For a medical emergency or immediate danger, call 911. For a sudden health change, contact the person’s health care team promptly.
Clarify the immediate plan with the care team
Ask who is coordinating care, what the next step is, and who can answer questions. If the person is in a hospital, ask to speak with the discharge planner or social worker.
Ask what support will be needed
Before a transition, ask about care needs, equipment, transport, follow-up appointments, and instructions. Tell the team what the household can and cannot realistically manage.
Request clear written instructions
Ask the team to explain medicines, follow-up arrangements, and when and whom to call. Have them clarify instructions you do not understand rather than guessing.
Choose one communication contact
With the person’s permission, agree on who will share updates and who can help with practical tasks. Ask the Eldercare Locator about community support if needed.
One thing to remember
You can ask for explanations, written instructions, and help planning the next step.
